Annual Officeholder Financial Disclosure Statements
If you were a public officer at any time during a calendar year (even for a day), you must file an annual Financial Disclosure Statement between January 1 and January 31 of the following calendar year. If you were newly appointed to fill a vacancy in an office, you must file a Financial Disclosure Statement within 60 days of the date you take office.
For 2020, officeholder annual Financial Disclosure Statements must be completed and submitted entirely online via the Arizona Candidate Portal. The online Financial Disclosure application allows for expedited, guided filing, and allows users to complete each applicable section of the form by following prompts. The Financial Disclosure application retains user information for future filings, allowing you to edit information where necessary and eliminating the need to start from scratch each year you are required to file.

Please note: The years shown refer to when the Annual Financial Disclosure Statements were filed. They cover financial activity occurring during the previous calendar year.
Note: Residential addresses and names of minor children have been redacted for years 2016 and earlier. Effective January 1, 2017, public officers may provide either a home or work address. Additionally, if disclosure of the identity of the public officer's spouse or minor child would otherwise be required, a public officer may comply with the identification requirement by using the term "spouse" or "minor child", as applicable.
