Effective September 24, 2021, the Secretary of State's Office will no longer process Advance Directive documents.

Pursuant to SB 1352, recently enacted by the Arizona Legislature, the Advance Directive Registry has been moved from the Arizona Secretary of States Office to AzHDR, Arizona's Health Information Exchange.

Documents received after September 24, 2021, are being forwarded to AzHDR. Upon receipt of the documents, AzHDR will communicate directly with applicants to provide instruction on how to proceed.
